The UC Immunization Policy requires all newly-admitted incoming students to provide proof of vaccination or immunity to measles, mumps, rubella (MMR), pertussis (whooping cough), varicella (chickenpox), and screening for tuberculosis, prior to entering and enrolling at the University of California. The meningococcal conjugate vaccine (MenACWY) is required for students age 21 and younger.

In addition, newly admitted incoming students must be screened for tuberculosis risk by completing the TB risk questionnaire form on the Ashe Patient Portal. Students who have any “yes” answers on the questionnaire are at higher risk for TB infection and are required to submit documentation of either skin or blood testing for TB, done within the 6-month period preceding entry to UCLA. Those with a previous positive TB test are required to submit documentation of a chest x-ray done within the 6-month period preceding entry to UCLA.

As a condition of physical presence on campus, all students must provide proof of vaccination to COVID-19, including boosters or submit a request for exception at least two weeks before the first day of instruction. Students who fail to comply with the policy will be subject to an enrollment hold.

All students must submit their information electronically through the Ashe Center Patient Portal only. Students with a specific program exception should follow the instructions given to them by their program/department.

UC Immunization Policy Overview

There are 3 separate immunization requirements for UC Affiliates:

1) SARS-CoV-2 (COVID-19) Vaccination Program

  • This policy applies to all UC students, trainees, personnel and all others who work, live, and/or learn in any of the University’s locations or otherwise participate in person in University Programs.

  • Covered individuals are required, subject to limited deferrals, exceptions, and associated non-pharmaceutical interventions, to be fully vaccinated, including boosted (if booster eligible), against COVID-19 before physically accessing the University’s Locations and Programs.

  • There are limited exemptions for covered individuals.

2) Influenza (Flu) Vaccine Policy

  • The University of California issued an executive order on Oct. 8, 2021, requiring flu vaccination for all students, faculty, other academic appointees, and staff living, working, who will learn, live or work on-premises at a UC facility.

  • The requirements for Fall 2022 will be announced when they are confirmed, however we recommend vaccination annually for all students. One dose on or after August 1st and prior to the November 1st deadline.

3) University of California Immunization Policy

  • This policy currently applies to all incoming students (undergraduates, graduates, transfers, and re-admits) who are living, studying, or coming to campus for any reason.

  • Covered individuals are required to show proof of immunity against Measles, Mumps, Rubella, Varicella (Chicken Pox), TdaP and Meningitis ACYW (for those under the age of 21) and screening for Tuberculosis (TB) risk

  • The University of California allows for exemptions to these immunization requirements based only a medical condition that is a contraindication to a vaccination. (See Medical Exception Request) Please note: approved religious COVID-19 exemptions do not apply to the required vaccinations listed under the UC immunization policy.

If you are living, studying, or coming to campus for any reason and don’t meet these requirements, an enrollment hold will be placed and you will not be able to add/drop or register for classes..Please make these immunizations a high priority!

Campus Deadline

All required vaccines (except Flu) should be completed before you come to campus. However, the official deadline is October 1st. Please consider the deadline to be "as soon as possible" so you don’t risk enrollment holds.

COVID-19 vaccines are free and available to all.

The COVID-19 vaccine and additional booster dose is now available for all students at The Ashe Center.

  • Due to COVID-19 precautions, no walk-ins will be accepted. Schedule using The Ashe Center Patient Portal by logging in (below) and selecting Appointments > COVID-19 Vaccine Appointment

How To Comply With The UC Immunization Requirements:

  • Submit your Statement of Intent to Register SIR/SLR.

  • Please allow 2 business days after submitting your SIR/SLR before attempting to access the Ashe Patient Portal.

  • Be prepared! Gather all of the info you’ll need to complete your requirements online.

  • Go to the UCLA Immunization Requirements website and sign in to The Ashe Center Patient Portal.

  • Once you are logged in, from Home page, click “Complete My Immunization Requirement.”

  • Enter all of the requested dates and immunization information.

  • Upload images or PDF files of your immunization records, laboratory tests, and/or chest x-ray by clicking on the Immunization Document upload tab.

  • You can check your compliance status from the Patient Portal by selecting Medical Clearance.
