Nursing students must adhere to the UCLA School of Nursing’s Immunization Policy and the public health policies, rules, and regulations at assigned clinical sites; both those operated by the University of California and its affiliates, so that they may provide healthcare services. If there are any objections or barriers to compliance, the student must immediately notify the Associate Dean for Student Affairs.
- Updated COVID-19 vaccine or declination
- Positive MMR titer OR proof of receipt of 2 doses of MMR vaccine (first dose needs to be on or after first birthday).
- Positive Varicella titer OR proof of receipt of 2 doses of Varicella vaccine (first dose needs to be on or after first birthday).
- Positive Hepatitis B surface antibody titer (HBsAb): Students not already immune to Hep B must have at least 2 doses of the Hep B series before class starts (or show proof of positive Hepatitis B surface Antigen). The entire Hepatitis B requirement must be completed by the first day of the spring semester or you will not be able to continue. Note: Some of our affiliated hospitals require proof of vaccine series in addition to the positive titer. Please include these in your upload documentation if possible.
- Negative IGRA (T-Spot, QuantiFERON®-TB test) within the 6-month period preceding entry to UCLA. If the result is positive, proof of negative Chest X-Ray is required.
- Tdap: 1 dose after age 11 AND Td or Tdap booster within the last 10 years.
- Meningococcal conjugate (Serogroups A,C,Y & W 135): Only for students under 22 years old: 1 dose on or after age 16.
- Seasonal Influenza vaccine given after August 1st of entering year.
- Physical exam (Note: Physical exam is $75 prior to start of classes, then $30 without UC SHIP, $0 with UC SHIP).
How to Comply with the UC Immunization Requirements:
- Submit your Statement of Intent to Register SIR/SLR.
- Please allow 2 business days after submitting your SIR/SLR before attempting to access the MyStudentChart.
- Be prepared! Gather all the information you will need to complete your requirements online.
- Go to the UCLA Immunization Requirements website and click on MyStudentChart.
- Sign into MyStudentChart using the "Sign in with your MyUCLA Logon ID" button.
- Once you are logged in, from the Home Page, click the Menu bar on the left-hand side. Scroll down and choose “Questionnaires.” Then, complete the Immunization Clearance, which also contains the annual TB testing requirement.
- Answer all required fields and upload supporting immunization records, including images, PDFs, laboratory tests and/or chest x-rays. The Immunization Worksheet for Outside Providers may be submitted in lieu of original immunization records but must be completed by your primary care provider.
- Once completed, it will need to be verified, and you can check your compliance and verification status online by logging in and selecting “Immunization Clearance Report” in the Menu drop-down list.
- When reviewing these reports, you’ll notice the “Overall Clearance Status” at the top. This will indicate whether you have submitted all required documentation—for example, it may say “Satisfied.” However, this does not confirm whether the information has been verified.
- To check verification status, you’ll need to scroll to the bottom of the form and look for the “Immunization Compliance Verification” section. This is the definitive source for whether a student is officially cleared. Here's what to look for:
- “All requirements reviewed and verified. Student is cleared.”→ All items have been submitted and verified.
- “All submissions reviewed and verified. Student not cleared – requirements still pending.” → Documentation has been reviewed, but some items are still missing.
- “None”→ The submitted documentation has not yet been reviewed. Please allow two weeks to complete.
- Once the verification status has been updated to “All requirements reviewed and verified. Student is cleared” you can print the report by click on the big print icon on the top left corner, a pop-up window will confirm your login information again.
- After logging in, in the header it will say "Print this page". This will allow you to print or save to PDF the Immunization Clearance Report to submit as needed.
IMPORTANT:
- New undergraduate nursing students need only comply with the general UCLA undergraduate requirements. Nursing-specific requirements will be due prior to starting your clinicals (for freshmen: year 3, transfers: year 2). The School of Nursing Clinical Placement Coordinator will send more information on how to submit the requirements closer to the start of clinical rotations.
