How do I submit my Medical Exception form?

Have your primary care provider complete the Medical Exception form.

Once the form is completed, log into MyStudentChart by opening the Immunization Clearance Questionnaire and choosing the Medical Exception option for the related immunization. Then, upload the supporting Medical Exception form with your questionnaire submission.

Once you submit your medical exemption request, the status will update automatically in your Immunization Clearance Report (under Menu in MyStudentChart). It’s not necessary to contact our office — we’ll reach out only if your submission is selected for further review.

All submissions are subject to audit, so please upload only appropriate documentation. Submitting unrelated or false files (like pictures of pets or memes) can delay processing or lead to disciplinary action.

What if the vaccine/titer is not available in my country?

You should still complete all other vaccines and/or testing, and submit your documents by the formal deadline. If you are missing any of the required immunizations, you can schedule an appointment at The Ashe Center.
