Medical & Religious/Personal Belief Exceptions
Students with a medical condition having a contraindication to the MMR, Varicella, Tdap, or Meningococcal conjugate vaccines will need their provider to complete the Medical Exception form. Likewise, students requesting a Religious/Personal Belief Exception will need to submit a Religious/Personal Belief exception request.

Once the form is completed, log into MyStudentChart, navigate to “Questionnaires” in the Menu drop-down list and choose the Medical Exception or Religious Exception option for the related immunization. Then, upload the supporting Exception form with your questionnaire submission.
Note: Once you submit your Exception request, the status will update automatically in your Immunization Clearance Report (under Menu in MyStudentChart). It’s not necessary to contact our office — we’ll reach out only if your submission is selected for further review.
All submissions are subject to audit, so please upload only appropriate documentation. Submitting unrelated or false files (like pictures of pets or memes) can delay processing or lead to disciplinary action.
COVID-19 and Influenza Vaccine Declination
To submit a voluntary declination for the COVID-19 or Influenza vaccine, log into MyStudentChart and select "Declination" in the Immunization Clearance Questionnaire to fulfill this requirement.
Program Exceptions
Students who are a part of the following self-supporting programs do not have access to The Ashe Center, including MyStudentChart, and are NOT subject to the immunization requirements:
- EMBA (Executive MBA)
- GEMBA (Global Executive MBA)
- ELP (Education Leadership Program)
- MPHHP (Master of Public Health for Health Professionals)
- MSE (Master of Science Engineering) 100% Online
- EMPH (Executive Master of Public Health)
- MHA (Master of Healthcare Administration)
- Applied Geospatial Information Systems and Technologies MAGIST
- Data Science in Biomedicine MS
All other incoming students from all remaining programs are subject to the full immunization requirements.
